Biliary hamartoma is a rare benign malformation of the bile ducts. We report a 62-year-old man who experienced mild epigastric discomfort and low-grade fever for several days. A series of laboratory and imaging examinations, including ultrasonography and computed tomography (CT), suggested the probable occurrence of microabscesses in the liver. However, follow-up sonography was not compatible with the patient’s improved clinical condition after empirical antibiotic treatment. The patient then underwent liver biopsy, and the final diagnosis of focal biliary hamartoma with nonspecific inflammation was made. We discuss the imaging features of this unusual case and review the literature.
