Neglected unreduced traumatic posterior dislocation of the hip in adults is usually associated with fracture of the ipsilateral femur or tibia and dislocation or fracture of the contralateral hip following a motor vehicle accident. The sequelae of a delay in reduction may contribute to the high incidence of avascular necrosis of the femoral head, posttraumatic arthritis, and sciatic nerve injury. In this report, we present the preliminary results of the surgical treatment of a posterior fracture–dislocation of the hip 2.5 months after the initial injury. At the 12-month follow-up, the hip had normal function, with no sign of arthrosis or avascular necrosis of the femoral head.
