Symptomatic bowel malrotation in adults is rare. We report an 88-year-old female presented with intermittent periumbilical abdominal pain, abdominal fullness and nausea for 2 days. Computed Tomography (CT) of abdomen revealed small bowel encircling the central superior mesenteric artery (SMA) in a whirl pattern. The patient underwent surgical correction
with Ladd’s procedure and a fi nal diagnosis of midgut volvulus with partial intestinal obstruction was made.
