Spontaneous spinal subdural hematoma is a rare entity, and cases are usually managed as surgical emergencies. We de-scribe a 45-year-old female with sudden onset of severe low back pain followed by radicular pain. At first, acute hernia-tion of the lumbar disc was suspected. Further evaluation with magnetic resonance imaging showed hematoma in the tho-racic subdural space. The patient underwent nonsurgical management with uneventful recovery. This report suggests there is a role for conservative management of selected cases of sponatenous SSDH. The etiology, precipitating factors, clinical features, and management are discussed.
