Intussusception is a common cause of abdominal pain in infants and children. Although most cases are idiopathic, about 10% of cases have a pathologic lead point, especially in elderly patients. Diffuse large B-cell lymphoma is not a common etiology of intestinal intussusception, and is a particularly rare etiology of intestinal intussusception in adolescent. We report a case in a 17-year-old adolescent who initially presented with ileocolic intussusception that mimicked acute ap-pendicitis. Exploratory laparotomy disclosed ileocecal diffuse large B-cell lymphoma as the pathological lead point. This case shows that the age of the patient and anatomic location of the intussusception have an important relationship to the etiology of this disease.
